# How Long Should Someone Stay in Treatment?

![stormy skies over arizona mountains](https://www.pivotrecoveryliving.com/hubfs/60%20day%20stay.png)

The call comes in on a Tuesday or Wednesday. It’s a weekday, after something happened and the feeling sharpened. His voice sounds more grounded than it did in August. He uses words like “processing” and “growth” with the ease of someone who has been in a lot of groups. And then he says it: *I think I’m ready. I’ve been here two months. I feel like I’ve got it now.*

You listen. You want to believe him. You have watched him change in ways that still catch you off guard, and a piece of you wonders if you are the one holding on too tight.

But here’s the thing: sixty days is not the point at which the work is done. It is the point at which the brain first becomes confident that it is.

### **How Long Should Someone Stay in Transitional Living?**

Ninety days is the research threshold for treatment to produce lasting change. NIDA’s[Principles of Drug Addiction Treatment](https://chestnut.org/Resources/95484007-fab2-4c88-9906-9781030ef31f/2018-NIDA-Principles-of-Drug-Addiction-Treatment.pdf) states it plainly: participation for less than 90 days is of limited effectiveness. For longer-term stability, the evidence points toward six months to a year. A 2024 study in *Addictive Behaviors Reports* tracking residents of Los Angeles [sober living houses](https://pmc.ncbi.nlm.nih.gov/articles/PMC11252607/) found a mean length of stay of 130 days among those who completed follow-up, and abstinence rates more than doubled between entry and the six-month mark, from 26 percent to 57 percent.

That number, 130 days, is what the data looks like when people stay long enough.

### **Why 60 Days Feels Like Enough (But Isn’t)**

Early recovery has a built-in confidence spike that mimics readiness.

Around the six-to-eight-week mark, the nervous system has restabilized. Sleep is better. Mood is more regulated. Guys who could barely hold a conversation in week one are now charming, articulate, and visibly more themselves. They look like they have it all figured out. He feels like he has it all figured out.

But neurologically, the prefrontal cortex is still catching up. The same brain region responsible for impulse control, long-range planning, and accurate self-assessment continues developing into the mid-twenties. In early recovery, it is also still re-establishing the dopamine pathways that were disrupted by use. The subjective experience of “I’m ready” at sixty days is real. The accuracy of that assessment is not.

### **What Happens to Young Men Who Leave Early?**

A 2025 study published in *Substance Use and Misuse* followed 352 [residents of sober living houses after they left](https://pmc.ncbi.nlm.nih.gov/articles/PMC12083549/) and tracked outcomes at twelve months. The research found that residents who left due to relapse had significantly lower abstinence rates and were less likely to be stably housed a year later. Residents who left because they were genuinely ready for independent living had the best outcomes across every measure: higher abstinence, fewer psychiatric symptoms, stable housing.

The distinction tells you something about the question your son is asking. “I want to leave” and “I am ready to leave” are different statements, and sixty days is almost never the moment when they converge.

The young men who do well at twelve months are not the ones who left when they felt ready. They are the ones who stayed until the environment confirmed it.

### **What to Do When Your Son Wants to Leave**

What you can do is hold the timeline without apologizing for it. Ask him to describe what six months from now looks like in specific terms: where will he live, what will he do for work, who is in his support network, what happens the first time something does not go the way he planned. The quality of his answers will tell you more than any reassurance will.

Most programs, Pivot included, have a phase structure for a reason. Progress through the phases is not arbitrary; it maps to observed changes in autonomy, regulation, and accountability, not just to the calendar. If his team is seeing the same readiness he is describing to you, that will show up in where he is in the program.

If it is not showing up yet, that is the answer.

**Frequently Asked Questions**

**How long should someone stay in a sober living program?** Research points to a minimum of 90 days, with significantly better outcomes at six months and beyond. NIDA identifies 90 days as the threshold below which treatment is of limited effectiveness. For young adults, where the developmental stakes are higher, six to twelve months is the evidence-based target.

**Is 60 days in sober living enough?** For most people, no. Sixty days is typically when early recovery confidence peaks, which can make leaving feel like the right call when it is often the riskiest one. The neurological and behavioral changes that make lasting recovery durable require more time to consolidate.

**What if my son is doing really well at 60 days?** Doing well at 60 days is a good sign and worth acknowledging. It is not the same as being ready for unstructured independent living. The goal of extended care is not to manage the crisis but to build the infrastructure that makes the next decade viable. That work takes longer than two months.

**How do I talk to my son about staying longer?** Focus on specific plans rather than abstract arguments. Ask him to describe in concrete terms what life looks like after he leaves. Then ask the program team what they are seeing. The two conversations together will give you a clearer picture than either one alone.
